Reporting for the older people old medical history and checkup details are done in reporting review which is generally done by the different machines and by use of technology. For reporting the paper, email and other method of correspondence are used as per the requirements and easiness. Trigger tool is the tool in care homes used for the record review of the old people for a target medical history; this is either done manually or automatically by the system present there. This is because old people are not getting proper care at the care homes in anyway (Ferner R.E, 1995).
The above analysis shows that at the care homes the error occurred because of the absence of the correct process and skilful people who take care of the older people. In home cares this problem also occurred due the number of old people and less nursing staff. In addition of this the medicine management system is also not so good that it can be managed efficiently and the medicines can be given to the old people timely as pert their problems. The previous research on the medication error at the care homes for the older people shows that more than 60 percent care homes having trouble in caring the older persons and getting them proper attention. (Nelson S et al, 2009)
People in care homes are a frail and vulnerable population at particular risk from medication errors, and it is a cause for concern that two thirds of care home residents in this study were exposed to one or more errors. For each event involving prescribing, dispensing or administration of a medicine, there was an 8%–10% chance of an error happening and a 14% chance of a monitoring error. Safety is a systems issue, and we believe this is the first study to consider the whole system of medication use in care homes; our simultaneous collection of qualitative data has allowed us to understand the causes of error and suggest solutions. (Hawksworth GM et al, 1999)
While considering the all types of issues in care homes the list goes increases. And also the medications errors that relates to some of the health organisations too. There are many such above problems existed at the big health care’s organizations. For example in UK, USA, and India etc. countries the health institutes are in number having good medical facilities for caring to the person but not have that kind of facilities at the care homes. Although the government in many countries are taking these issues very seriously but the problem is continuing day by day. The older people also should have some knowledge about the health care issues and they should be able to take care of that by themselves. For that these care homes must train them well or make them learn how to prevent initial diseases. (Kelly WN et al, 2009)
The research done in many care homes at USA, resulted in many good side and bad side of the issue. But one good thing was that number of care homes participated in the analysis report. The most important part was not having the proper medical facilities in many of the care homes and also the local government not helping much them in getting the required resources.
